About Your Bill & Insurance
Insurance plans such as Medicare, Medicaid and commercial insurance will cover some of the costs of your hospitalization after your deductible is paid. You will be responsible for paying any deductible and charges not covered by your insurance. If you have questions about your bill or your insurance coverage, contact the Financial Counselor at 574-946-2106, or Patient Accounts at 574-328-9532, from 8 am – 4 pm.
A number of medical services are billed separately from your hospital bill. These fees are not set or controlled by PMH. Services providing a separate billing statement are listed below. Call their phone numbers for billing inquiries.
A patient may ask for an estimate of the amount the patient will be charged for a nonemergency medical service provided in this facility. The law requires that an estimate be provided with 5 business days.
Notice of Availability of Charity Care
In recognition that all patients who seek healthcare from PMH may not have the financial resources or insurance coverage to afford care, PMH will make a reasonable amount of uncompensated services available to persons meeting the eligibility requirements. Eligibility for charity care will be limited to persons whose family income is not more than the current poverty guidelines established by the US Department of Health and Human Services. Persons whose income is greater than the guidelines are eligible for hospital charity care at a discounted, sliding fee scale.
No one will be denied access to services due to inability to pay.
