Community Health Needs Assessment

Pulaski Memorial completes a Community Health Needs Assessment every three years to identify and prioritize the community’s health concerns.

Below are the results from the last two Community Health Needs Assessments and PMH’s implementation plan for each assessment. These surveys are vital to assisting PMH in fulfilling our mission of serving our patients and community.
