Pulaski Memorial Hospital is pleased to announce the launch of its 2025 Community Health Needs Assessment, a comprehensive initiative aimed at identifying and addressing the most pressing health needs of our local communities.
“Community feedback is essential,” said Hospital CEO Steve Jarosinski. “PMH will be gathering input from residents, local organizations, community leaders, and even other area healthcare professionals to better understand the health challenges and opportunities across the region.

This process began on March 4th when PMH hosted leaders from around the area to start discussing important topics affecting the community. Leading that discussion were experts from the Indiana Rural Health Association (IRHA). Hospital leaders once again partnered with the IRHA to help with the project because of their experience assisting rural hospitals. “This is our fifth time completing the Community Health Needs Assessment and our third with IRHA assisting,” said Jarosinski. “They have been a great partner for us and have worked with several rural Indiana hospitals similar to PMH to complete their Health Needs Assessments.”
Over the next month, the survey will be accessible online by clicking the link below.

Hard copies of the survey will be available at the admission desks in the hospital and at all six clinic locations throughout Pulaski, Starke, and Marshall Counties. You can also access the survey by scanning the QR code in this story. Once all the surveys are collected and analyzed, IRHA will provide a written report to PMH, with the results publicly reported on the Hospital’s website.
Mr. Jarosinski encourages everyone 18 years or older to complete the survey: “The information we gather from this process often directs our strategic planning for the next several years. The public’s participation will assist us in achieving a thorough review of the health status and the unmet health needs in our community.”

The Community Health Needs Assessment survey is available until May 17th. If you have any questions, please contact Brian Ledley, Vice President of Communications and Patient Experience,at (574) 946-2148 or via email at bledley@pmhnet.com.
